Financial Consultation

Information for Admission

Paradise Park Assisted Living and Memory Care is licensed and regulated by the Illinois Department of Public Health ("IDPH"), under the Assisted Living and Shared Housing Establishment Code. This Code requires certain documentation be secured prior to admission. If necessary, our staff can assist you in obtaining the proper paperwork so that we may make the move-in process easy as pie!

* Furniture that is comfortable and practical
* Pictures, collectables, anything that says, "Home".
* Clothing appropriate for the season and outings
* Blankets, pillows, afghans
* Linens and towels;

REQUIRED DOCUMENTATION:
* Current History and Physical (within 120 days of move-in);
* A Physician's Certification Form, stating that the Resident is appropriate for placement;
* A complete list of all current medications;
* Copies of Social Security, Medicare, and Insurance cards;
* Any Advanced Directives (Living Will, Power of Attorney, etc.)
