for Admission
Paradise Park is licensed and regulated by
the State of Illinois through the Department of Health. This means
residents and their families are part of a pro-active assisted living community. However, it also
means there is a little paperwork to be completed to ensure
we meet the State’s requirements. More importantly, our forms help us meet our resident’s needs
and expectations. If needed, we are available to assist the resident
or family in their preparation of these forms. We are here to
help in every way we can.
PREPARATION FOR ADMISSION
The following information is necessary
to complete the admission process:

WHAT TO BRING
Paradise Park is an assisted living community that supports our residents
in maintaining dignity and independence in a beautiful, warm,
home-like atmosphere. Resident suites at Paradise Park are unfurnished,
which allows each resident to express his or her own uniqueness
and personality. If the resident desires, Paradise Park will furnish
a resident’s private room with bed linens, including
pillows, for a twin size bed and towels, washcloths and shower curtain
for their private bathroom. Residents may want to bring some
of the following items: personal toiletries, bedspreads, pillows,
bath rugs, wall hangings, radio, television, telephone, VCR, DVD
player or custom window treatment. Residents may also want to bring some
of the following furniture: bed, recliner chair, rocker, love seat, bookshelf,
dresser, display shelf or chest of drawers. We will assist you with
hanging your pictures for your room. Please feel free to measure the room for furniture placement or window treatment.
