Information for Admission

Paradise Park Assisted Living and Memory Care is licensed and regulated by the Illinois Department of Public Health (“IDPH”), under the Assisted Living and Shared Housing Establishment Code. This Code requires certain documentation be secured prior to admission. If necessary, our staff can assist you in obtaining the proper paperwork so that we may make the move-in process as seamless as possible.

REQUIRED DOCUMENTATION:

Current History and Physical (dated no more than 120 prior to move-in)
A Physician’s Certification Form, stating that the Resident is appropriate for placement
A complete list of all current medications
Copies of Social Security, Medicare, and Insurance cards
Any Advanced Directives (Living Will, Power of Attorney, etc.)

WHAT TO BRING

Paradise Park provides:

Window blinds
Microwave and mini-fridge (In Assisted Living Suites Only)
Ceiling light, night light, and bathroom light
Toilet paper and Kleenex
Laundry Soap / Fabric Softener
Depends and incontinence products (at an additional charge)
